These are my medical records from a hospital where I've been undergoing treatment for the last two years.

I haven't requested these in order to make a complaint, but because I'm transferring treatment to another place. This is the quickest way to expedite that, but involves completing a witnessed form, paying £50 odd and waiting 40 days. But now I have them, they make for interesting reading!
