New Falls Service helps avoid 289 hospital trips

Between November 2014 and end of March 2015, our new Better Care Fund Falls Service has helped 289 patients aged 65 or over to get treatment at home rather than have to go to hospital.

GPs and East Midlands Ambulance Service can make referrals to support people who fall at home or in the community.

The phone line provides access to local community health services for patients who have been assessed as not needing to go to hospital.  There is a rapid handover for ongoing care and support - avoiding admissions to the hospital.  The service is provided to the patient in their own home and is available 24/7
