I've put off todays task so effectively I got the deadline moved. But it had to happen. 

Recalling and (massivly over) analysing events that were rightly and wrongly never discussed.
Left with a sense of guilt thats not mine to carry, but I will anyway for a while. Peace that I've done what I can. 
Mostly a total conviction that systems and protocols are important but they should never ever forget about the person. Even or especially if that person is easy to forget about. 
That individual with eyes and a heart, stories and dreams, battles we dont know about, a laugh, a favourite place, friends.
Dr Suess got it right
