In the air eventually after the engineer who was meant to pull the widget out of the widget holder got frustrated, gave it a good yank, and left the little bit at the end of the widget still in the widget holder, where it remained until the second engineer came along some hours (seemed like) later with his widget holder clearer and removed the offending widget end.
