30/365 - Not known?

I had a check up yesterday & they made me fill in this form - you fill it in yourself, so I'm not sure why the choices across the top are 'male' 'female' and 'not known' - surely there could be a comment space instead where you could fill in details if, for example, you were having gender reassignment - 'not known' just seemed an odd choice!
